Back Pain Treatment in Jacksonville, FL

A Jacksonville Injury Clinic for Back Pain

Back pain can start with a car accident, a work injury, a lifting strain or no clear cause at all. At East Coast Injury Clinic, medical doctors (MDs), advanced practice registered nurses (APRNs), chiropractors, physical therapists and in-house nerve testing work from one shared record, so your evaluation, treatment and paperwork stay in one place.

Our chiropractors include chiropractic neurologists: Doctors of Chiropractic (DCs) with post-graduate training in neurology. Our physical therapy team builds rehab plans, and a visiting interventional pain physician sees patients whose pain continues despite conservative care.

Most new episodes of low back pain improve within a few weeks. Our job is to check for anything that needs more than time, treat what we find and help you get back to work, driving and sleep. Call (904) 513-3954 to schedule at our Southside clinic on Deerwood Park Blvd or our North Jacksonville clinic on N Main St.

How Your Lower Back Works and What Causes Back Pain

Your lower back, or lumbar spine, has five vertebrae (L1 to L5) that sit on the sacrum. Between the vertebrae are intervertebral discs, each with a tough outer ring called the annulus fibrosus and a gel-like center called the nucleus pulposus. Small facet joints link the vertebrae at the back, and muscles and ligaments hold everything together. Spinal nerve roots leave the spine at each level and travel into the hips and legs.

Pain can come from any of these structures. Common causes we evaluate include:

  • Muscle strain or ligament sprain: the most common source of acute back pain, often after lifting, twisting or a sudden jolt.
  • Herniated or bulging disc: disc material pushes beyond its normal border and may irritate a nerve root. See herniated disc treatment.
  • Sciatica: pain that travels from the lower back or buttock down the leg, usually from nerve root irritation. See sciatica treatment, including how it differs from lower back pain.
  • Facet joint irritation: pain that is often worse when leaning back or twisting.
  • Degenerative disc disease and spinal stenosis: age-related disc changes, or narrowing of the spinal canal that can cause leg pain with standing or walking.
  • Spondylolisthesis: one vertebra slipping forward on the one below it.
  • Sacroiliac (SI) joint pain: pain low in the back or buttock where the spine meets the pelvis.
  • Injury from a crash or a job: rear-end collisions, falls from equipment and repetitive lifting can strain the back or injure a disc. A pinched nerve can follow.

How We Evaluate Back Pain

Your first visit starts with a detailed history and exam. Your provider may check:

  • Posture, range of motion and movement patterns to see which motions bring on your pain
  • A neurological exam of reflexes, strength and sensation in the legs
  • The straight leg raise test and other tests that help show whether a nerve root is irritated
  • Red flags that need urgent care or a different specialist

If you have numbness, tingling or weakness, EMG and nerve conduction (NCV) testing in our clinic can help show whether a nerve is involved. These tests are often more informative about three weeks or more after symptoms start, so your provider will advise on timing.

Imaging such as an MRI is not routinely needed for low back pain unless there are red flags or symptoms are not improving. When imaging is needed, we refer you to an outside imaging center and review the results with you. We do not have MRI on site.

Non-Surgical Back Pain Treatment Options

The American College of Physicians’ 2017 guideline recommends starting acute and subacute low back pain care with non-drug options such as superficial heat, massage and spinal manipulation, and it emphasizes exercise and rehabilitation for chronic low back pain. Your plan may include:

  • Chiropractic care: spinal adjustments using the Diversified Technique or the low-force Activator Method, which may help reduce pain and improve motion.
  • Physical therapy and therapeutic exercise: core stabilization, mobility work, graded walking and lifting mechanics, designed to rebuild strength and help lower the chance of flare-ups.
  • Soft-tissue work: myofascial release and stretching for muscle guarding and spasm.
  • Interventional pain care: if pain continues despite conservative care, our visiting interventional pain physician may evaluate you for procedures such as epidural steroid injections or facet joint injections. Learn more about pain management.
  • Surgical referral: if your exam or imaging points to a problem that may need surgery, we refer you to a spine surgeon.

What we don’t do: we do not perform surgery or MRI scans in-house, and not every back problem is one we should treat. If your symptoms suggest a fracture, an infection, a kidney problem or another condition outside our scope, we will tell you and refer you. Recovery takes time, and individual results vary.

Back Pain Symptoms and When to Get Help

Back pain can feel dull, sharp, burning or stiff. It may stay in the lower back or travel into the buttock or leg. Consider an evaluation if:

  • Your pain lasts more than a few days or keeps coming back
  • Pain travels below the knee, or you have numbness or tingling in a leg
  • Pain started after a car accident, a fall or a work injury
  • Pain is keeping you from work, sleep or daily activities

Go to the ER or Call 911 If You Have

  • Numbness in the groin, genitals, buttocks or inner thighs (saddle numbness)
  • New trouble urinating, loss of bladder control or loss of bowel control
  • Leg weakness that is getting worse, foot drop or trouble walking
  • Back pain with fever or chills, or unexplained weight loss
  • Back pain after major trauma, or after a minor fall if you have osteoporosis
  • New back pain with a history of cancer

Cauda equina syndrome is compression of the bundle of nerve roots at the bottom of the spinal canal, and it is a medical emergency. Saddle numbness, new bladder or bowel changes and progressive leg weakness may be signs of it. Do not wait for a clinic appointment. For more, read red flags of low back pain.

Back Pain After a Car Accident or Work Injury

Back pain is one of the most common injuries after a crash, and it can start hours or days later. In Florida, your own Personal Injury Protection (PIP) coverage usually pays first. Three rules matter:

  • Start care within 14 days. PIP pays medical benefits only if initial care begins within 14 days of the accident (Fla. Stat. 627.736).
  • The EMC finding: medical benefits can go up to the $10,000 PIP limit, once a medical doctor, osteopathic physician, PA or APRN makes an Emergency Medical Condition (EMC) determination. A chiropractor cannot make this determination. Our MDs and APRNs evaluate crash patients and make that clinical judgment based on the exam.
  • The $2,500 limit: without an EMC, PIP medical benefits are limited to $2,500.

Frequently Asked Questions

What type of doctor should I see for back pain in Jacksonville?

For new, mild back pain, many people start with a primary care provider. If pain lingers, follows an injury or comes with leg symptoms, a clinic that combines medical and musculoskeletal care can help. At East Coast Injury Clinic, MDs and APRNs perform medical evaluations, chiropractors and physical therapists treat the injury, and a visiting interventional pain physician is available when conservative care is not enough. Go to the ER first for emergency warning signs.

When is back pain an emergency?

Go to the ER for numbness in the groin or inner thighs, new bladder or bowel problems, leg weakness that is getting worse, back pain with fever, or back pain after major trauma. These can be signs of cauda equina syndrome, infection or a fracture, which need emergency care.

Do I need an MRI or X-ray for back pain?

Often not at first. Imaging such as an MRI is not routinely needed for low back pain unless there are red flags or symptoms are not improving. If your exam calls for imaging, we refer you to an outside imaging center and review the results with you. We do not have MRI on site.

How long does low back pain usually last?

Most new episodes of low back pain improve within a few weeks. Pain that lasts longer than about 12 weeks is usually called chronic, and guidelines emphasize exercise and rehabilitation for that stage. Recovery time varies with the cause and your activity level, and individual results vary.

What should I avoid when my back hurts?

Long periods of bed rest, heavy lifting with twisting and sitting for hours without a break often make back pain worse. Gentle movement such as short walks is usually better than staying in bed. Your provider can tell you which activities are safe for your specific injury.

Can a chiropractor help with lower back pain?

Spinal manipulation is one of the non-drug options the American College of Physicians recommends for acute and subacute low back pain. Many patients report less pain and better motion with chiropractic care, especially when it is paired with exercise. Individual results vary, and our chiropractors work alongside medical providers who screen for problems that need other care.

Is it back pain or sciatica?

Back pain mainly stays in the back. Sciatica is pain that follows the path of the sciatic nerve into the buttock and leg, often with numbness or tingling. Read more on our sciatica page, which also compares lower back pain and sciatica.

Can a car accident cause back pain days later?

Yes. Back pain, stiffness and leg symptoms can appear hours or days after a crash. For Florida PIP to pay medical benefits, initial care must begin within 14 days of the accident, so it is reasonable to get evaluated promptly. See our car accident injury clinic page.

Do I need a referral to be seen for back pain?

No referral is needed to schedule your first visit. Call (904) 513-3954. If you were seen at an ER or urgent care, bring that paperwork.

How is my care paid for at East Coast Injury Clinic?

Most injury care at East Coast Injury Clinic is covered through one of four paths: auto insurance PIP after a car accident, a personal injury attorney’s Letter of Protection, state workers’ compensation for a work injury, or federal workers’ compensation (OWCP or Longshore) if you are a federal or maritime employee. We also see patients under Medicare, Tricare, VA Community Care, and a limited number of private plans. Our front desk verifies coverage before your first visit and explains exactly what is and is not covered for your specific case, so there are no surprises. Call 904-513-3954 or visit eastcoastinjury.com to start.

Which location should I visit for back pain?

We have two Jacksonville clinics: Southside at 10550 Deerwood Park Blvd, Suite 609, Jacksonville, FL 32256, near JTB and I-295, and North Jacksonville at 13453 N Main St, Suite 302, Jacksonville, FL 32218. Provider schedules vary by location, so call (904) 513-3954 and our front desk will help you choose.
