What Is Whiplash?

Whiplash happens when a sudden force moves the head backward and forward (extension and flexion) faster than the neck muscles can brace. The strain can affect the muscles and ligaments of the cervical spine, the small facet joints between the vertebrae, the intervertebral discs and, in some cases, the nerve roots that travel into the arms. Clinicians often call the resulting group of symptoms whiplash-associated disorder, or WAD.

Whiplash Grades (WAD 0 to IV)

The Quebec Task Force classification is a common way clinicians describe how severe a whiplash injury is:

Grade 0: no neck complaints and no physical signs.
Grade I: neck pain, stiffness or tenderness, with no physical signs on exam.
Grade II: neck complaints plus musculoskeletal signs, such as reduced range of motion and point tenderness.
Grade III: neck complaints plus neurological signs, such as weakness, reduced reflexes or numbness in the arms.
Grade IV: neck complaints with a fracture or dislocation. This needs emergency and specialist care.

The grade helps your provider decide which tests make sense and which members of the team should be involved.

Common Causes of Whiplash

Car crashes: rear-end collisions are the classic cause, but side-impact and head-on crashes can also cause whiplash. How you feel does not always match how much damage the vehicle has. See our car accident injury clinic for what to do after a crash.
Work injuries: commercial driving, forklift and equipment jolts, or a load that shifts suddenly. If you were hurt on the job, Florida workers’ compensation or federal workers’ compensation (OWCP or Longshore) may apply.

Whiplash Symptoms

Whiplash symptoms often appear hours to a day or two after the injury, and some people notice them later as stiffness sets in. Common symptoms include:

Neck pain and stiffness, and trouble turning your head to check a blind spot
Headaches that start at the base of the skull
Shoulder and upper back pain, or tightness between the shoulder blades
Pain, numbness or tingling in an arm or hand, which can point to an irritated nerve root (a pinched nerve)
Dizziness or balance problems
Jaw pain or clicking (TMJ)
Fatigue and trouble sleeping
Trouble concentrating, light sensitivity or feeling foggy, which can also be signs of a concussion
Anxiety about driving, irritability or flashbacks after a crash

Go to the ER or Call 911 First If You Have

Severe neck pain after a high-speed crash or rollover, or pain directly over the spine
New weakness, numbness or tingling in the arms or legs, trouble walking or clumsy hands
Loss of bladder or bowel control
Loss of consciousness, confusion, repeated vomiting, a headache that keeps getting worse, slurred speech, a seizure or one pupil larger than the other
A sudden, severe headache or neck pain with dizziness, double vision, trouble speaking or swallowing, facial drooping or sudden falls. These can be signs of an injury to an artery in the neck (cervical artery dissection) or a stroke.
Trouble breathing, or chest or abdominal pain
A head injury while you take blood thinners

How Long Does Whiplash Last?

There is no single timeline. Many people improve over the first few weeks to months, while some have neck pain, headaches or stiffness that last longer. Recovery tends to depend on the grade of the injury, how much pain and disability you have early on, and other factors such as sleep, stress and the physical demands of your job. How We Evaluate Whiplash

History and exam. An MD, APRN or chiropractic neurologist asks how the injury happened, where you were sitting, where your headrest was and what you feel now. The exam checks neck motion, tenderness, strength, reflexes and sensation, and screens for red flags.
Imaging when it is needed. X-rays or an MRI are ordered when your exam or a clinical decision rule such as the Canadian C-spine rule points to them. We do not have MRI on site. We refer you to an imaging center and review the results with you.
Nerve testing for arm symptoms. An EMG (electromyography) test measures electrical activity in muscles, and a nerve conduction study (NCV) measures how well signals travel along a nerve. Together they can help show whether a nerve root in the neck is irritated. These tests are often more informative about three weeks or more after an injury, so your provider will advise on timing.
Balance testing for dizziness. Videonystagmography (VNG) records eye movements to help assess the inner ear and balance system.
Concussion screening. The same forces that cause whiplash can cause a concussion, so we check for it. See concussion care.
PIP Emergency Medical Condition (EMC). After a car crash, our MDs and APRNs can determine whether you had an EMC for Florida PIP. That finding is a clinical judgment based on your exam, and we cannot promise one in advance.

Whiplash Treatment Options

Your plan depends on your grade, your exam findings and your goals. It may include:

Education and staying active. Guidelines generally favor gentle, guided movement over prolonged rest. Your provider explains what to expect and which activities to change for now.
Chiropractic care. Our chiropractors may use spinal manipulation or mobilization, including the Diversified technique or the low-force Activator Method, once serious injury has been ruled out. Your chiropractor explains the benefits and risks of neck adjustments and uses low-force options when they fit better.
Physical therapy and therapeutic exercise. Range-of-motion work, deep neck flexor and upper back strengthening, posture and shoulder blade control, and a graded return to work, driving and sport.
Soft-tissue treatment. Myofascial release and stretching may be part of your plan. Florida PIP does not pay for massage therapy.
Vestibular rehabilitation for dizziness or balance problems.
Interventional pain management. If pain continues despite conservative care, a visiting interventional pain physician may evaluate you for procedures such as facet joint injections, medial branch blocks or radiofrequency ablation.
Post-accident psychiatry. Anxiety about driving, poor sleep and flashbacks are common after a crash. Our psychiatrist sees car accident patients in person in Jacksonville or by telepsychiatry.

These treatments are designed to reduce pain and restore function. Many patients report steady progress, and individual results vary.

What Whiplash Recovery Can Look Like

Care usually moves in phases. First, calm pain and protect your sleep. Next, restore neck motion. Then rebuild strength and endurance for work, driving, lifting and sport. Your provider re-examines you at set points and uses measures such as the Neck Disability Index (NDI) and pain scales to track progress and adjust the plan.

An honest note: some people recover in a few weeks and some need months of care, and a share of patients still have symptoms after that. We will tell you what we find, what we expect and when a referral to another specialist, such as a spine surgeon or neurologist, makes more sense.

Florida PIP and Whiplash After a Car Accident

The 14-day rule: PIP pays medical benefits only if you receive initial care within 14 days after the accident (Fla. Stat. 627.736). An emergency room visit counts.
The EMC finding: medical benefits can go up to the $10,000 PIP limit, once a medical doctor, osteopathic physician, PA or APRN makes an Emergency Medical Condition determination.
The $2,500 limit: if a treating provider determines you did not have an EMC, PIP medical reimbursement is limited to $2,500.
80% of reasonable charges: PIP pays 80% of reasonable charges for medically necessary care, within your policy limit.
No massage or acupuncture: Florida PIP does not pay for massage therapy or acupuncture.

Do I need an X-ray or MRI for whiplash?

Can a chiropractor help with whiplash?

How should I sleep with whiplash?

Many people sleep more comfortably on their back or side with a pillow that keeps the neck level with the rest of the spine. Sleeping on your stomach twists the neck and often makes symptoms worse. Your provider can suggest positions and pillow height based on your exam.

Can whiplash cause headaches or dizziness?

Yes. Headaches that start at the base of the skull are common after whiplash and may come from the upper neck (cervicogenic headache). Dizziness can come from the neck, the inner ear or a concussion. We may use a neurological exam and VNG balance testing to help sort out the cause.
